If you are e-filing a Form 1040, 1040A, or 1040EZE-filers must mail Forms 8453
- U.S. Individual Income Tax Declaration for an IRS e-file Return, to the Internal Revenue Service within three business days after receiving acknowledgement of return acceptance. An electronically transmitted return will not be considered complete until a Form 8453 signed by the taxpayer is received by the I R S.
Please do not mail multiple copies of each Form 8453 to the I R S and please keep their copy for your records. Per Publication 1345 (revised 11-2004), page 28, Electronic Return Originators may electronically image and store Forms 8453. When a PIN signature method was accepted by the I R S, DO NOT mail Form 8453 to the Submission Processing Center.
There are several kinds of form 8453 and the IRS makes the determination of which form to mail after you e-file. You have to follow what your e-filing confirmation e-mail says. If your e-mail says that your self-selected PIN was accepted, you don't have to mail anything to the IRS. If your e-mail specifically asks you to mail form 8453, then you must mail form 8453. You can print form 8879 for your records, but you do not have to mail form 8879.

If you are e-filing a form 1041
You must print and sign, and mail Form 8453-F to Internal Revenue Service Ogden Submission Processing Center (OSPC) Mail Stop 6052 Ogden, UT 84201
